It started with stains.
At first, the team in the Texas hospital’s Sterile Processing Department (SPD) thought the discoloration on a few instruments was detergent residue. They rewashed them—same result. Within days, more trays were coming out of the washer with dull or spotted finishes, and the OR staff began to complain. Something was wrong.
When Facilities joined the investigation, they checked the washers, filters, and reverse osmosis unit. Everything looked fine—until they reviewed the recent water quality log. Conductivity had spiked from 2 µS/cm to nearly 40 µS/cm.
That number told the story.
Conductivity measures the total amount of electrically charged impurities—ions—in water. High conductivity means elevated levels of dissolved solids such as chlorides, calcium, or iron that can corrode stainless steel or leave residues on instruments. According to ANSI/AAMI ST108, conductivity is one of the most important indicators of water purity and system performance. It’s a proxy for total dissolved solids and acts as an early warning that water quality is deteriorating—often before visible damage occurs.
In this case, the culprit was a seasonal change in the municipal water supply. Runoff had increased the mineral content of the incoming water, overwhelming the facility’s filtration system. Once identified, the team re-sanitized the system, replaced the exhausted filters, and implemented tighter conductivity monitoring to prevent future issues.
